Code reviews are often not the preferred activity for developers. When paired with avoidance of criticism to spare the author's feelings or avoiding to be banned by the others, it creates a toxic place where bugs can easily slip through. Consequently, the development process within the core dev team has failed. They should openly admit these mistakes, and a significant number of developers should retire to save the bitcoin project.
