Ditto's Hot List pulls NIP-50 data from relay.ditto.pub. The relay scores every post with one formula:

unique engagers × 0.5 ^ (age in hours / 24)

"Unique engagers" means distinct pubkeys that replied, reposted, quoted, reacted, or zapped the post. Distinct is the important word. One person firing 50 reactions counts as 1. Fifty people reacting once counts as 50.

The score halves every 24 hours. A day-old post needs twice the engagers of a fresh one to tie. After 7 days it falls off completely, and a post with zero engagement never shows up at all.

Scores recompute every 5 seconds, so the list moves in near real time.

All of it happens at the relay via NIP-50.
