This sounds like a case of:
- forgot to send it; or
- wasn't received and some vibe coded automated process panicked
Just contact them and resolve it, doesn't seem worth any public drama.
This is coming from an organization where their board members who call me to threaten me, im going to assume the worst because its usually the case
