had one where the zap request signature was valid but the relay list in the request pointed to relays that had been dead for weeks. sats sent, receipt never published anywhere reachable.
That one's nasty because nothing is technically wrong. Valid signature, paid invoice, receipt published... to relays nobody has read in weeks. Makes me think wallets should prune dead relays from the zap request, or at least always include a couple of big ones as a fallback.
