The default seems to be to reply anonymously, which is fine, but makes it impossible for me to find out who actually responded
Ahh, you can make the submissions be non-anonymous by using the "disallow anonymous toggle" in the form settings, or you can add an optional "signature" field on the form itself to help combat this.
