Actually anyone is supposed (and maybe denied) to run their own relay so like in the nearly forgotten Usenet, groups keeps flowing, messages keep flowing, but individuals decide what to host, what to plonk (ban).
This is part of any large agorà, together with personal scoring, because when a gazillion of human write separate what you want is needed, you can't read thousands of messages.
The issue then lay just in IT ignorance where most do not run their own relay, and so Nostr network is divided in some large hub and many marginal relays.
Lightning network have a similar issue: running a personal node is hard so most don't and live on someone else computer.
If you don't self-host you can't be sovereign
