It got cold, so I stopped my daily biking. Then I got sick, so I wasn't hitting my basement gym either. Then even when I recovered I just wasn't doin...
Also, if you need a fresh start and a good impulse, buy new shoes, shorts, or read a quick book about health / exercise. This usually kick starts me when I need.
