when using @npub1marke... , is a buyer able to add a note to specify a variable in a product?

or does the seller need to create an entirely new item/product to account for a single variable change while everything else remains constant?

say I am selling cookies but i give them a choice between sugar or maple syrup for sweetener - can they add a note to their purchase that would inform me of their preference?
