Chiropractor + good chair. It's hard to fix if you don't stop slouching for many hours every day so you need to get a good chair and sit in it properly. Legs and hips at 90 degree angles.

Maybe not that severe, but I found regular stretching and kettlebell swings to really help my desk-life posture. I'm planning on adding regular dead-hangs, as I've heard good things about those lately.

I had a tattooer that would wear one of those posture back brace things. he also had this band-pike thing he'd velcrow around his stomach and lower back. I'm not sure if it was to help with posture or just prevent it from getting worse though.
