I was overhearing the wilderness medical lecture a couple days ago and I guess what happens is the last thing they do before letting go and falling off of the host is vomit into the host.
That’s where the real pathogen exchange happens.
It could take 12 to 24 hours once they attach to you before they get their fill, puke and drop off.

If you get them off carefully (no squeezing) before that you shouldn’t be at risk.
