we have nip-04 which are end-to-end encrypted but leak metadata. We have a newer spec (nip17) which hide metadata and are also end-to-end encrypted. We are working on a new group encryption spec that is efficient for large groups but its experimental.

You can always set up a private relay for you and your friends.
