My top scheme to organize my accounting, medical and other files that I have been using for 10+ years:
YYYY-MM-DD-NameOfInstitution-Name-of-document.pdf

I have it in folders, etc, but having the sortable date in the document name has been such a time saver that just keeps compounding and getting more useful over the years. Anytime I download a file, I manually rename it to this.
