Wouldn’t paid relays solve for this? The censored would pay the fee to those relays willing to accept and broadcast that message.
Which relay is going to transmit notes of a terrorist organization? Which client is going to allow relays that transmit such notes?

Chatgpt think that having user pay 1 sats to send a note might lead to spam, but not charging them would not?

I doubt the network would haven taken off with that economic model, but yes the beneficiary of the service paying for the service is a viable model.

Bitcoin would work if miners had to charge out-of-band for mining transactions. It would just be less eficcient. If we could do like Bitcoin on Nostr we would, but Nostr doesn't have a native currency...
