That last line about institutions refers to places for the mentally I'll/people unwilling or unable to perform basic cares. They had issues, but less than what we do now.
Yep, same issue here in Canada. We closed all the institutions because they were deemed cruel, as if people freezing and screaming at imaginary people in the street is any less cruel and undignified
