It’s because a minority have access to Starlink, but only a small minority of that minority have the technical expertise to set up VPNs capable of working alongside the government’s whitelisted internet and routing traffic outside it.
However, running these VPNs and even buying them is a risk. There have already been arrests and confiscations of equipment in such cases.
