@nprofile1q... a quick check suggests that if you have control of the senders and they could run exim you can set SNI on the client connection, use a direct tls connection and use haproxy-like traffic router to direct to appropriate mx

And given the single sender, multiple recipient nature of SMTP I don’t see how you could reliably configure the SNI any other way.
